Home Connections program provides home-based case management and support to patients facing serious, progressive illness and their families.
The Medical Social Worker provides assessment and case management services to Home Connections patients and families. Provides appropriate referrals to community programs. Responsibilities include home visits and telephone contact. Performs other duties as requested
Master's Degree in Social Work from a school accredited by the Council on Social Work Education and the NYS Education Department. NY State license and current registration required. Training or clinical counseling experience, at least one year in home care or a health related field required. Experience with community resources/agency systems needed. Strong clinical skills needed in the areas of assessment, lifestyle change, crisis intervention, problem resolution and coping issues, long-term planning, community resource coordination/information/referrals, and in patient/family/agency advocacy. Proficiency in oral and written communications is essential. Spanish speaking helpful.
A valid New York State drivers license and a car available for use when on-duty.
