This position handles the day to day coordination of the team clinical functions; assists in implementing the patient plan of care with staff, volunteers, and outside agencies. Communicates, as a liaison, between staff, patients, and outside agencies. Assists with maintaining records. May involve direct patient care.

Essential Duties and Responsibilities:

  • Assists in coordination and implementing of the patient plan of care.
  • Handles phone calls from the patient, primary care giver, or designee regarding any questions or concerns.
  • Collaborates with the interdisciplinary teams, RNs, or Team Manager.
  • Coordinates and schedules patient visits.
  • Provide timely documentation/data entry per Hospice time frames and deadlines.
  • Communicates current health status and needs of patients to appropriate team professionals.
  • Assists the team and Team Manager with other administrative functions as necessary.

Qualifications:

  • Education / Certification

Graduate of an accredited School of Practical Nursing with current LPN licensure in the State of New

York.

  • Experience

1 year of nursing practice as a Licensed Practical Nurse preferred.
