Provides case management services, under the supervision of a an MSW, to patients and families utilizing a variety of modalities, and in accordance with State/Federal regulations. The social worker is an adjunct member of the interdisciplinary team and acts as a support and provides assistance with facilitating paperwork / applications to help meet patient needs related to remaining safe in their home or transition to a higher level of care (placement). 

Essential Duties and Responsibilities:

  • Provides case management services under the supervision of a an MSW, including support, advocacy, long term care planning, community resource coordination and other services to optimize patient care and safety.
  • Assists, records, and reports the patient and family participation in collecting and completing community applications for patient services / benefits needed (i.e. MLTC / LTC Medicaid, Medicare, Social Security, Social Services). 
  • Assists team members with accessing community resources to improve and meet patient care needs. 
  • Assists the case assigned Medical Social worker complete patient transfers to a higher level of care placement needs to maximize quality of life.
  • Adheres to Social Work Quality Assurance standards, keeps abreast of social work trends and knowledge for service provision; and assists in audits/peer reviews as directed by his/her supervisor.
  • Participates in the agency's quality improvement program and documents quality assurance data as required; Completes work, care and documentation with accuracy and within time frames.
  • Maintains client charts and other written/computerized record keeping assignments in a timely fashion.  

Qualifications

Education / Certification

Has a baccalaureate degree in social work from an institution accredited by the Council on Social Work Education; or a baccalaureate degree in Psychology, sociology, or other field related to social work and is supervised by an MSW as described in paragraph (b)(3)(i)(A) of the federal regulations for hospice; or

Has a baccalaureate degree in social work from an institution accredited by the Council on Social Work Education; or a baccalaureate degree in Psychology, sociology, or other field related to social work; and has one year of experience in a health care setting. If the BSW is employed by the hospice before December 2, 2008, he/she is exempted from the MSW supervision requirement.

Experience

  • One year of social work experience in a health care or community setting; or
  • Knowledge of Medicaid application process and required paperwork for accessing Long Term Care benefits.
