Home Connections Mission is to help ease patients through the course of serious illness, regardless of changes in diagnosis, medical treatment or location. 

The Home Connections Manager is responsible for the daily supervision of care and services provided to patients. Manages and coordinates with the interdisciplinary team of the Home Connections Program and is responsible for the clinical direction and overall supervision of staff on interdisciplinary team. Plans and executes program development and outreach strategies.

Essential Duties and Responsibilities:

  • Handles implementation of care to meet the physical, psycho-social and spiritual needs of the patient/family while maintaining standards of care consistent with program goals; verifiable through outcome measures such as Patient Satisfaction Survey, Utilization Measures, and Clinical Outcomes.
  • Handles recruiting, hiring, evaluating and terminating personnel and making staffing pattern recommendations to assure adequate, appropriate and clinically competent staffing levels are maintained.
  • Maintains current working knowledge of state and federal regulations as they apply to Licensed Home Care Services Agency and is able to articulate same to staff, including Hospice and CHHA eligibility requirements.
  • Prepares staffing schedules as appropriate and anticipates staffing profile needs proactively. Assures 24/7 nurse on-call availability for both phone and direct patient contact.
  • Collaborates with the Chief Nursing Officer and Director of Quality Improvement in the implementation of Home Connections Improvement Program and implements in multiple monitoring activities that are both patient and organizational focused to promote clinical quality, corporate and regulatory compliance.
  • Uses leadership position to set positive, attainable expectations, objectives and goals for staff development using the performance management interview or similar process.
  • Provides guidance and support to staff, in the planning of patient care, to ensure that such care is rendered as components of an interdisciplinary team and handles any novel and diverse patient care problems that arise, including appropriate risk management interventions.
  • Assists in the preparation of the annual budget.
  • Promotes and assures and documents a collaborative, efficient and effective IDG Team meeting that is focused on both patient-family goals of care and optimal symptom management.
  • Meets quality improvement standards relative to chart audits in order to assure both regulatory and patient care protocol compliance.
  • Assures the timely completion and return of all initial, renewal and modified orders and reviews all PPOC's for procedural compliance.
  • Adheres to HIPAA regulations and policies. Assures the security of patient electronic/written records.
  • Manages, instructs and directs those who supervise and or deliver patient care in order to assure staff adheres to job duty performance in the provision of service to patients.
  • Elicits no founded patient/family complaints or any preventable or unusual occurrences.
  • Adheres to requirements of both CHPC and Caring Hearts Home Care policies and procedures that are both patient focused (clinical) and organizational focused (administrative).
  • Maintains an appropriate schedule of supervisory field visits, and assures that staff adheres to yearly DOH mandatory requirements.
  • Plans and participates in the orientation, mentorship and staff development needs of personnel to assure that program personnel are current in clinical information and practices.
  • Manages, instructs and directs those who supervise and/or deliver patient care, including PT, OT, SLP, MSW, Nutrition, RT, RN, and LPN to assure high quality job performance in the service to patients.
  • Responds to clinical on-call needs with appropriate interventions and staff guidance.

Qualifications:

  • Education / Certification

Graduate of an accredited school of nursing with current RN licensure in the State of New York.

BSN preferred. 

Certification in Hospice and Palliative Care Nursing preferred.

Valid Driver's License, free of serious violations, and vehicle required for use when on duty or on-call.

  • Experience

Two (2) years of clinical nursing experience required; Hospice experience preferred.

Management Experience required.
