The Director of Quality and Process Improvement is responsible for the overall agency wide continuous quality improvement program that is both patient and organization focused to ensure quality benchmarks are met or exceeded across the organization. Assists with ensuring compliance with all applicable Federal, State and local laws, rules and regulations as they apply to the organization. Designs a quality strategic plan that includes annual goals and focused assessments. Leads performance improvement teams teaching analytic tools to support assessment and plans. Works closely with the VP of Clinical Services, Corporate Compliance Officer, Director of Education and clinical Directors.

Essential Duties and Responsibilities:

  • Hiring, mentoring, evaluating, and separating department staff.
  • Assists in the development of the department annual budget including monthly variance analysis.
  • Maintains up to date knowledge of all applicable Federal and State regulations following up to ensure agency compliance with all changes.
  • Directs all activities pertaining to quality monitoring.
  • Responsible for collating and analyzing data to identify trends working with QI and clinical teams on root cause analysis.
  • Collaboratively creates action plans for improvement including follow up evaluation of data and organizational performance in meeting or exceeding quality benchmarks to ensure consistent improvement.
  • Provides monthly quality metric reporting inclusive of any noted trends.
  • Coordinates and oversees completion of annual agency-wide systematic review and annual policy review
  • Plans and oversees the Quality Assurance and Quality Assessment Performance Improvement committee meetings.
  • Participates in the CHPC Board Quality sub-committee meetings reporting quality metric data at board meetings as requested.
  • Responsible for coordinating, reporting, and distribution of all satisfaction surveys for both Hospice and palliative care programs ensuring improvement agenda based on trends.
  • Responsible for gathering data from unusual occurrences and complaints and summarizing trending at designated timeframes for reporting in line with regulatory requirements.
  • Collaborates with education department for development of quality and regulatory educational programs.

Non-Essential Duties and Responsibilities:

  • Education/Certification

Graduate of an accredited school of nursing with current RN licensure in the State of New York.

BSN or MSN preferred

Quality related certification preferred

  • Experience

Three to five years clinical nursing experience preferred

Management experience helpful

Experience in healthcare quality improvement and leading successful performance improvement plans

Experience in Hospice & Palliative Care preferred

Experience working in various healthcare settings helpful
