Full time.  Monday-Friday.    

Provides assessment and grief counseling services to adults and children whose loved one received Hospice care.  Facilitates support groups and presents educational programs to CHPC.

Essential Duties and Responsibilities:

  • Formulates a counseling plan of care and appropriately initiate and facilitate clients through the counseling process.
  • Meets productivity guidelines established by CHPC and external funding sources
  • Identifies community resources to meet the client's needs and facilitates linkage with those resources.
  • Maintains client charts and other written/computerized record keeping assignments in a timely fashion.
  • Participates in individual supervision, clinical case conference, and interdisciplinary care team meetings.
  • Education / Certification

NYS License, Master's Degree in Counseling or Social Work field from an accredited college or university

Valid Driver's License, free of serious violations, and vehicle required for use when on duty.

  • Experience

2 years experience in a counseling position, preferably in the field of grief and loss. 

Masters level student internship (1 academic year) in a grief counseling agency can be substituted for 1 year experience.
